Portage Pop Warner is accountable to its parent organizations, and we require participants to complete and submit forms exactly as directed before being allowed to participate.

2023 POP WARNER Participant Contract & Parental Consent Form
REQUIRED.
All signatures and dates must be completed. 

2023 POP WARNER Physical Fitness & Medical History Form 
REQUIRED.

Football & Cheer | REQUIRED.
Section II must be completed and signed by a licensed physician and dated January 1, 2022 or later.
This form must be submitted with ORIGINAL signatures from the physician and a date of January 1, 2022 or later.  
We cannot accept a copy.

2023 SCHOLASTIC DOCUMENTATION 
REQUIRED.

CURRENT, FULL-YEAR REPORT CARD. DO NOT SEND POWER SCHOOL REPORT. 
REPORT CARD MUST HAVE THE FULL SCHOOL YEAR OF GRADES FOR 2021-2022

ALL  PARTICIPANTS:
AN ORIGINAL BIRTH CERTIFICATE WILL BE REQUIRED TO BE PRESENTED TO A DISTRICT HLA MEMBER  PRIOR TO POST SEASON GAMES TO VALIDATE OUR COPY ON FILE.
Please ensure you have a valid ORIGINAL Birth certificate available.

Please do not send an original, but ensure the copy is of an official birth certificate.
A "Mother's Copy" is NOT a birth certificate.
Birth certificates are available from the county/state of birth.
